The two issuers that publish this ask for a Japanese number you can be reached on — they do not ask for a mobile. A landline counts.

Could you read a Japanese application form, speak Japanese to the issuer, and write the form in your own hand?

All three, because one issuer asks for all three: you must understand and speak Japanese and write the form 自筆 — in your own hand. A helper is fine for the bank account, not for that card. Answering only for the written form would put an application you cannot complete into your "fits" list.
